About the Role

We are seeking an experienced and compassionate Support Worker to join our Independent Living Services Team. As a Support Worker, you will play a vital role in providing high-quality support to young people with complex needs, empowering them to live independently and achieve their goals.

Key Responsibilities
  • Provide person-centered support to young people with complex needs, promoting their independence and well-being.
  • Develop and implement tailored support plans, working closely with the young person, their family, and other professionals.
  • Assist with daily living tasks, such as personal care, meal preparation, and household management.
  • Support young people to access education, employment, and leisure activities, promoting their social and emotional well-being.
  • Collaborate with the team to ensure seamless handovers and effective communication.
Requirements
  • Experience working with vulnerable adults or young people with complex needs.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with young people, families, and colleagues.
  • Ability to work flexibly, including evenings, weekends, and sleep shifts.
  • Resilience and patience, with a calm and caring demeanor.
  • Knowledge of safeguarding policies and procedures, with a commitment to ensuring the safety and well-being of young people.
