Job Overview

Main Duties
  • Deliver person-centred care to patients with dementia, focusing on their physical, emotional, and social needs.
  • Work collaboratively with the multidisciplinary team to develop and implement individualised care plans.
  • Provide emotional support and guidance to patients and their families, promoting independence and well-being.
  • Contribute to the development of the service, identifying areas for improvement and implementing changes to enhance patient care.
Requirements

We are looking for a highly motivated and enthusiastic individual with experience in dementia care. You will need excellent communication and organisational skills, with the ability to work in a fast-paced environment.
