Position Overview

We are seeking a dedicated and skilled registered nurse to become a vital member of our specialized nursing team. This role focuses on delivering exceptional, evidence-based rheumatology care, offering monitoring, support, and education to our patient population through nurse-led clinics, while collaborating closely with medical and allied health professionals.

Work Environment

The position is primarily based at Norfolk and Norwich University Hospital, with opportunities for outreach clinics.

Employment Type

Part-time: hours per week

Key Responsibilities

As the successful candidate, your responsibilities will include:

  • Delivering a comprehensive service for patients within the specialized care area.
  • Providing direct patient care and support to enhance quality of life by educating patients on symptom management and control following diagnosis.
  • Serving as a resource for other healthcare professionals.
  • Maintaining and promoting high standards of professional care while embodying and influencing the values of the Trust.
  • Developing, implementing, and evaluating care plans for a specialized patient group.

We welcome enthusiastic nurses from diverse nursing backgrounds, particularly those with experience in rheumatology nursing. While experience is desirable, training will be provided.

Job Responsibilities

  • Managing a specified caseload of patients, providing information and support to empower patients in making informed decisions about their care.
  • Collaborating closely with Consultants, medical and nursing staff, and the multi-disciplinary team.
  • Ensuring patients are seen within recommended timeframes and targets.
  • Visiting inpatients as necessary to ensure continuity of care and provide advice/support.
  • Accepting referrals from medical/nursing staff for newly diagnosed and existing patients.
  • Periodically reviewing caseloads and discharging patients appropriately.
  • Responding to telephone inquiries and coordinating admissions or appointment changes as needed.
  • Conducting comprehensive assessments of patients' symptoms and organizing necessary investigations.
  • Administering medication according to established guidelines.
  • Effectively communicating sensitive information to patients and caregivers in a clear and timely manner.
  • Liaising with other Specialists, Consultants, GPs, Dieticians, and various agencies.
  • Working autonomously, managing and prioritizing your workload based on patient needs.
  • Assisting the Line Manager in providing clinical and managerial leadership.
  • Deputizing for senior colleagues as required.

Qualifications

  • Registered Nurse on the appropriate part of the NMC Register.
  • Possession of, or working towards, a relevant Degree qualification.

  • Significant post-registration experience, including relevant specialty experience.

  • Strong communication and interpersonal skills.
