About the Role

Norfolk Primary Care C.I.C. is seeking an experienced clinician to join our Home Visiting Service (HVS) team. As an Advanced Nurse Practitioner, you will provide high-quality care to housebound patients on behalf of their registered GP Practice.

Key Responsibilities

  • Provide clinical assessment, diagnosis, treatment, and referral services to patients in their homes.
  • Work collaboratively with GP Practices to meet the needs of patients and support the delivery of policy and procedures.
  • Demonstrate safe and effective clinical decision-making, including assessment and diagnostic skills.
  • Provide nurse leadership and support to the HVS team as required.

Requirements

  • Registered with the NMC / HCPC and meet the requirements for Advanced Practice.
  • Experience in primary care and home visiting services.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
