About the Role

Norfolk Care is seeking an experienced Care and Support Specialist to join our Reablement Intervention Team. As a key member of our team, you will play a vital role in supporting individuals to regain independence and achieve their goals for independent living.

Key Responsibilities

  • Work in teams of two to support rapid hospital discharges and provide person-centred care to enable individuals to regain independence.
  • Conduct dynamic risk assessments and deliver care with an outcomes-focused attitude.
  • Liaise with other healthcare professionals to signpost service users and family members to additional support, equipment, and services.
  • Develop and maintain relationships with service users, family members, and other stakeholders to ensure high-quality care and support.

Requirements

  • Previous experience in providing person-centred care, ideally qualified to Level 3 or equivalent.
  • Ability to work 7 days per week on a 4 on/3 off shift pattern.
  • Full driving licence and access to a vehicle.
