Key Responsibilities:- Deliver high-quality patient care as part of the clinical team, ensuring the highest standards of care and safety.
- Undertake My Plan and Review/CDPF for nurses within the immediate team, and/or support registered nurses to undertake appraisal of more junior staff, resulting in individual development plans.
- Facilitate the development of nursing practice through specific projects aligned with place-based objectives.
- Act as a managerial and clinical resource for services within your remit, providing guidance and support to staff.
- Coordinate patient activity and promote a wide team approach to patient care, ensuring seamless delivery of services.
- Regularly oversee care to ensure staff are providing the highest standard of care and identify training or development needs.
- Participate in clinical supervision and promote the clinical supervision of junior staff, ensuring a culture of continuous learning and improvement.
- Deliver and participate in formal and informal teaching to reflect the needs of the service, promoting best practice and evidence-based care.
- Regularly participate in observations of clinical practice to monitor standards of practice and create bespoke developmental plans for staff within your team.
- Support the Clinical Nurse Manager with investigations into incidents and complaints, ensuring a fair and transparent process.
- Ensure the voice of the user is heard and facilitate high-quality patient and carer feedback, promoting a patient-centered approach to care.
- Coordinate the service, including allocation of staff, responding to queries, and liaising with the coordination hub regarding referrals and allocations.
- Registered Healthcare Professional on the NMC register.
- Experience caring for patients with palliative care needs.
- Collectively reflect on and assess the impact of person-centered ways of working within the multidisciplinary team.
- Understand the benefits of working with a patient-centered philosophy of multidisciplinary team working.
- Incorporate knowledge of life-limiting conditions and best practice evidence into care planning and decision-making.
- Experience of formal and informal teaching.
