About the Role

We are seeking an experienced and skilled Community Nursing Sister/Charge Nurse to join our team at Berkshire Healthcare NHS Foundation Trust. As a key member of our community nursing services, you will be responsible for leading a team of nurses in providing high-quality care to patients in their own homes.

Main Responsibilities
  • Lead a complex and diverse caseload, working to prevent hospital admissions and support patients in their preferred place of care.
  • Maintain accurate and confidential patient records, adhering to trust policies and procedures.
  • Manage a defined caseload of patients, assessing care needs, developing and implementing care programs, and evaluating their effectiveness.
  • Accountable for the standards of care and determining the activity of your team.
