Job Summary

Home Group is seeking a compassionate and dedicated Supported Living Worker to join our team. As a Supported Living Worker, you will play a vital role in empowering individuals with complex needs to live independently in the community.

About the Role

  • Assist customers with daily living tasks to promote independence and achieve their goals.
  • Support customers with personal care and medication needs, as outlined in their support plans.
  • Carry out risk assessments, support planning, and regular reviews to ensure the best possible outcomes for customers.
  • Work collaboratively with colleagues to provide a comprehensive support service.
  • Use technology to update support plans, complete online learning, and collaborate with colleagues.

Requirements

  • A Level 2 Health and Social Care Certificate or willingness to work towards it.
  • Experience of caring for others, either in a professional or personal capacity.
  • Ability to work on own initiative, remain calm under pressure, and have a resilient approach.
  • Flexible and personal approach, with a focus on listening to customers' needs and recognizing their individuality.
  • Valid UK/EU driving license and over 25 years old to comply with Motability insurance criteria.
