About the Role

We are seeking a dedicated and compassionate Support Worker to join our team at Active Care Group. As a Support Worker, you will play a vital role in enabling our clients to engage in rehabilitation, activities of daily living, and recreational activities, while empowering them to be as independent as possible.

Key Responsibilities
  • Provide individualized support to clients with complex needs, including those with epilepsy, global developmental delay, autism, and ADHD.
  • Assist clients with daily living tasks, such as personal care, meal preparation, and medication management.
  • Develop and implement personalized rehabilitation plans to promote independence and quality of life.
  • Collaborate with therapists and other healthcare professionals to ensure comprehensive care.
  • Maintain accurate records and reports to track client progress and outcomes.
Requirements
  • Full, clean UK driving license.
  • Experience working with children and young adults with physical and cognitive impairments.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to work varied shifts, including weekends and evenings.
