Job Overview

We are seeking a highly motivated and experienced Senior Clinical Pharmacist to join our Community Mental Health Team. As a key member of our team, you will be responsible for providing clinical pharmacy services, including medication reviews and clinical input, to patients with complex medication needs.

Main Responsibilities
  • Attend multidisciplinary team meetings to discuss patient care and treatment plans.
  • Prioritize patients with complex medication needs for medication histories, medication reviews, and clinical input.
  • Provide pharmaceutical care to patients, including prescription monitoring and safe use of medicines.
  • Act as an expert source of pharmaceutical advice for healthcare staff, patients, and carers.
  • Collaborate with healthcare teams to ensure seamless care and optimize patient outcomes.
Requirements
  • Pharmacist registration with the General Pharmaceutical Council.
  • Postgraduate diploma or master's qualification in clinical pharmacy or equivalent experience.
  • Experience of delivering clinical pharmacy services, including prescribing.
  • Experience of working in mental health and community care.
  • Excellent communication and interpersonal skills.
