Job Title: Advanced Practitioner

This is an exciting opportunity to be part of a dynamic team and contribute to the development of a new service from its inception. Working hours will be within practice contract hours of 8am and 6pm.

Main Responsibilities
  • Work within own competence and professional code of conduct as regulated by the NMC, ensuring high-quality patient care.
  • Produce accurate, contemporaneous, and complete records of patient consultations, adhering to legislation, policies, and procedures.
  • Prioritize, organize, and manage own workload to maintain and promote quality, ensuring efficient use of resources.
  • Initiate and participate in the maintenance of quality governance systems and processes across the organization and its activities.
  • Utilize the audit cycle to evaluate the quality of work, implementing improvements where necessary in partnership with other clinical teams.
  • Collaborate on improving healthcare quality, responding to local and national policies and initiatives as appropriate.
  • Evaluate patients' response to healthcare provision and the effectiveness of care.
  • Use a structured framework to manage, review, and identify learning from patient complaints, clinical incidents, and near-miss events.
  • Assess the impact of policy implementation on care delivery.
  • Understand and apply legal issues supporting the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patients' health procedures and local guidance.
About Us

Leigh Primary Care Network supports a patient population of approximately 57,000, comprising 9 practices in Leigh (Wigan Borough). Our network has successfully worked together for several years, but the formation of PCNs has allowed us to develop further and establish a successful workforce, including First Contact Practitioners, Care Coordinators, Community Link Workers, and Pharmacists, led by an effective leadership team headed by our Clinical Director and Data & Transformation Lead.

We now want to add an ANP service to the team, believing it will be instrumental in continuing to meet the challenges in primary care and providing high-quality and responsive services in Leigh.

  • Provide effective care planning and comprehensive assessments.
  • Support the development of effective systems and processes to ensure well-coordinated, high-quality care that meets contractual requirements.
  • Commitment to maintain mandatory and role-specific training.
  • Ability to assess and manage risk when working with individuals.
  • Knowledge of EMIS Web and System One Clinical System.
