Position Overview

The role of Clinical Nurse Specialist (CNS) within the Solihull Healthcare Partnership (SHP) team is essential for delivering comprehensive care to patients with both immediate healthcare needs and those requiring ongoing support. We welcome applications from experienced CNS candidates with specialized interests in areas such as acute care, geriatric care, and mental health, as well as those with a more generalist background.

As a seasoned nursing professional, you will operate within your defined professional scope. Your expertise will enable you to conduct initial clinical assessments through telephonic or digital triage, and when necessary, facilitate in-person evaluations. You will gather patient histories and perform clinical assessments to establish diagnoses, treatment plans, and evaluate patient care outcomes. Your role will involve making informed clinical decisions and providing expert care to patients within the SHP framework. You will collaborate with a broader clinical team on-site, with opportunities to seek guidance or escalate complex health issues as needed.

Key Responsibilities

The primary objective of this position is to assist the SHP team in delivering exceptional care to our patient community while enhancing their overall experience. In this capacity, you will contribute to service enhancements and share insights on improving patient services and workforce satisfaction. Your responsibilities will also include mentoring and developing both clinical and non-clinical team members, reflecting our commitment to professional growth at SHP.

You will work in partnership with a multidisciplinary general practice team to address patient needs, ensuring adherence to established policies and procedures while providing nursing leadership when necessary. We seek highly motivated CNS candidates with independent prescribing capabilities to join our team. This role requires you to deliver timely, high-quality, patient-centered clinical care, addressing patient needs with the utmost adherence to evidence-based practices.

Your duties will encompass follow-up care for patients needing further evaluation, managing referrals, and processing results from diagnostic tests and medication requests.

Clinical Responsibilities

  • Effectively triage patients through online forms or telephone consultations.
  • Assess, diagnose, plan, implement, and evaluate treatment for patients presenting with minor illnesses or undifferentiated diagnoses.
  • Conduct assessments via telephone or in-person, ensuring the most suitable environment for patient evaluation.
  • Manage treatment plans for patients with complex needs and proactively identify those at risk of developing long-term conditions.
  • Educate patients on health promotion strategies and lifestyle modifications, utilizing your prescribing authority to ensure safe and effective medication management.
  • Follow up with patients as necessary based on their healthcare needs and resource utilization.
  • Prioritize health issues and intervene appropriately in urgent or emergency situations, including initiating emergency care when required.
  • Book diagnostic tests as indicated and refer patients to other services as appropriate.
  • Review pathology reports, interpret results, and take necessary actions based on findings.
  • Complete medical reports for various agencies, ensuring compliance with relevant guidelines.

  • Maintain accurate and complete records of patient consultations in line with legislation and organizational policies.
  • Deliver care in accordance with NICE guidelines and evidence-based practices.
  • Engage in self and peer review to assess the effectiveness of care delivery.
  • Participate in quality governance initiatives and collaborate with other clinical teams to enhance healthcare quality.
  • Support training and shared learning across the practice and organization.
  • Understand and apply legal frameworks related to safeguarding vulnerable individuals.
  • Act as a nursing advisor to other members of the healthcare team.
  • Encourage teamwork and collaboration within the practice.
  • Contribute to quality improvement and service development initiatives.
