About the Role

eSift Ltd is seeking a highly skilled and dedicated Community Connector to join our team. As a Community Connector, you will play a vital role in supporting the development and mobilization of integrated primary care mental health services.

Key Responsibilities
  • Provide early intervention and promote early triage, assessment, and formulation of need and care plans for patients with mental health presentations.
  • Coordinate and support patients to access a wide range of community services and resources, including social care, housing, family debt, and employment counseling.
  • Work collaboratively with a range of professionals across Primary Care Networks, including GPs, Practice Nurses, Mental Health Practitioners, Pharmacists, and Clinical Psychologists.
  • Take a multidisciplinary approach to support the ongoing development of integrated primary care mental health services.
Requirements
  • A Diploma or degree in an appropriate subject, such as Counselling, Psychology, Mental Health, Social Work, or Health and Social Care.
  • Good communication skills, both written and verbal.
  • Proficiency in Microsoft Word and Excel.
  • Ability to travel around the area required for the role, with own transport preferred.
