About the Role

We are seeking a highly skilled and dedicated Community Nurse to join our team at the Royal Devon and Exeter NHS Foundation Trust. As a Community Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their own homes and communities.

Key Responsibilities
  • Assess, plan, and deliver nursing care to patients in their own homes and communities.
  • Work collaboratively with other healthcare professionals to provide comprehensive care and support to patients and their families.
  • Develop and implement individualized care plans that meet the unique needs and goals of each patient.
  • Provide education and support to patients and their families to promote health and well-being.
  • Conduct regular assessments and reviews to ensure that patients are receiving the best possible care and support.
Requirements
  • Registered Nurse with a valid nursing license.
  • Experience working in community nursing or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Knowledge of community nursing principles and practices.
