Care Services Coordinator

Salary: £38,295 plus £3,000 vehicle allowance

Acuity Care Group Limited is a prominent specialist provider in services for individuals with Learning Disabilities and Autism across the UK. We cater to both adults and children with long-term support needs.

In the role of Care Services Coordinator, you will oversee the establishment and implementation of new care packages while ensuring the continuous operation of services tailored to a diverse range of individual needs, particularly in the Cornwall area. You will be supported by a dedicated team of Support Workers and will collaborate closely with the Operations Manager for your region.

As part of a regional team and reporting to the Operations Manager, this dynamic position involves regular interaction with individuals, their families, Clinical Commissioning Groups (CCGs), and healthcare professionals to guarantee that person-centred care remains a priority and that compliance standards are consistently met.

To excel in this position, you should possess a genuine commitment to delivering exceptional care standards and a desire to positively impact the lives of those we support. Strong leadership abilities and experience in learning disabilities, autism, mental health, and complex care are essential. The ability to thrive in a fast-paced environment, coupled with excellent time-management and communication skills, is crucial.
You will also be expected to participate in an on-call rota to provide out-of-hours support for both clients and staff in your area. Travel within your region and occasional visits to the head office will be necessary.
This role is expected to evolve further due to growth in the sector.

Key Responsibilities:

  • Recruitment and onboarding of Support Workers
  • Supervision and guidance of your teams
  • Participation in regular meetings with clients, CCG representatives, peers, and management
  • Rota management
  • Conducting risk assessments
  • Providing training to team members
  • Coordinating the establishment of new care packages
  • Managing incidents, investigations, disciplinary actions, and grievances
  • Delivering regular reports and updates
  • Providing shift cover to support clients

Required Skills, Knowledge & Experience:

  • Proven experience in managing multiple care packages, including rota management
  • In-depth knowledge of working with learning disabilities, autism, mental health, and complex care
  • Ability to conduct assessments and develop care plans and risk assessments
  • Experience in leading and managing a team
  • Exceptional communication skills
  • Capability to organize a busy workload and prioritize effectively
  • Familiarity with CQC regulations and compliance within the care sector
  • Possession of a Level 4 or 5 diploma in Health & Social Care
  • Ability to work independently as well as collaboratively within a team
  • Understanding of financial data and experience in budget management
  • A valid UK driving license and access to a personal vehicle
