In this role, you will provide essential support to clients through personalized sessions, care planning, risk evaluation, and collaboration with various agencies.
Your responsibilities will also include facilitating group sessions and coordinating with our partners to enhance each client's overall health and wellness.

We are dedicated to implementing an evidence-based, comprehensive psychosocial program alongside various activities that promote the 5 Ways to Wellbeing.
Our emphasis is on ensuring effective recovery care coordination within a fully integrated service model, focusing on successful and sustainable transitions from treatment.
