About the Role

We are seeking a skilled and compassionate Community Rapid Response Rehabilitation Worker to join our team at North Lincolnshire Home First Community Support. As a key member of our rehabilitation team, you will play a vital role in supporting individuals in their own homes, helping them to regain and maintain their independence.

Key Responsibilities
  • Provide rehabilitation support to individuals in the community, following an accident, event, or illness, to help them regain and maintain their independence.
  • Work in conjunction with other health and social care professionals to develop and implement personalized therapy plans.
  • Support with moving and handling, attending to personal care needs, and administering medication as required.
  • Maximize opportunities for social inclusion and emotional wellbeing.
  • Respond to crisis visits and provide welfare visits in a person's home to identify and address any further support needs.
Requirements

To be successful in this role, you will need to be able to work flexibly, including weekends and unsocial hours, including bank holidays, and be able to travel around North Lincolnshire. A 25 hour per week contract offers a great work-life balance, and as a member of our team, you will have opportunities for further development and progression.
