Job Overview

An exciting opportunity has arisen within South Tees Hospitals NHS Foundation Trust to join our Urgent Community Response (UCR) and Hospital at Home service. We are seeking skilled and outstanding senior clinicians to provide high-quality patient care and contribute to the growth and development of our team.

Main Responsibilities

As a Community Clinical Nurse Practitioner, you will be responsible for providing in-depth assessments of nursing care and treatment for patients in their own homes, preventing unnecessary admissions and facilitating timely discharge from hospital. You will work closely with a team of experienced clinicians to deliver high-quality patient care and contribute to the development of our service.

Requirements
  • Independent non-medical prescriber with demonstrable experience
  • Advanced physical assessment and disease management skills (level 7) or willingness to work towards level 7 within the first 2 years of appointment
  • Teaching and mentoring qualification
  • Diploma or degree awarded by a professional body
  • Registered as fit to practice by the Nursing and Midwifery Council (NMC) as a registered adult nurse
  • Professional knowledge acquired through degree supplemented by postgraduate diploma specialist training, experience, short courses, and further specialist training to masters (level 7) equivalent level
