Job Overview

An exciting opportunity has arisen within South Tees Hospitals NHS Foundation Trust to join our Urgent Community Response (UCR) and Hospital at Home service. We are seeking skilled and outstanding senior clinicians to contribute to the growth and development of our team.

Main Responsibilities

As a Community Clinical Nurse Practitioner, you will be a key member of our UCR service, providing in-depth assessments of nursing care and treatment for patients in their own homes. This will involve working closely with patients, families, and healthcare professionals to prevent unnecessary admissions and facilitate timely discharge from hospital.

You will be responsible for managing a caseload of patients with complex needs, utilizing your advanced clinical skills, non-medical prescribing, and case management approach to achieve positive outcomes. Experience of working in a community setting, emergency assessment unit, or rehabilitation would be an advantage.

Requirements
  • Independent non-medical prescriber with demonstrable experience
  • Completed advanced physical assessment and disease management skills course (level 7) or willing to work towards it within the first 2 years of appointment
  • Teaching and mentoring qualification
  • Diploma or degree awarded by a professional body
  • Registered as fit to practice by the Nursing and Midwifery Council (NMC) as a registered adult nurse
  • Professional knowledge acquired through degree supplemented by postgraduate diploma specialist training, experience, short courses, and further specialist training to masters (level 7) equivalent level
