Job Summary

Band 6 Community Sister: within Hambleton and Richmondshire District Nursing

2 x Hours per week

We are seeking a Registered Nurse with the Specialist Practitioner in District Nursing qualification to lead and manage a community nursing team within a multi-professional environment.

The post holder will be responsible for providing exceptional clinical care, innovative practice, and support to the Clinical Lead to our ever-developing teams.

This role will provide the opportunity to work closely with GP practices and other partners, in supporting patients with highly complex and often high acuity needs to prevent unnecessary hospital admission.

The successful candidate will be expected to adapt to the changing needs of the Trust and the community models of care as they arise.

About Us

South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff to the Trust; this training programme has been designed to support our leaders in developing their understanding of leadership and management skills.

The post holder will be expected to attend the leadership development programme, and the Quality Improvement Programme that the Trust delivers.

This programme aims to:

  • Explore leadership within the NHS
  • Promote Trust Values and Behaviours
  • Develop your Leadership effectiveness and skills
  • Practitioner level in quality improvement, equipping you with the skills to champion, lead and complete quality improvement within your role

After completing this four and a half days of training, you will be able to explore further leadership and improvement training opportunities.
