Job Overview

An exciting opportunity has arisen within South Tees Hospitals NHS Foundation Trust to join our Urgent Community Response (UCR) and Hospital at Home service. We are seeking skilled and experienced senior clinicians to contribute to the growth and development of our team.

Main Responsibilities

As a Community Clinical Nurse Practitioner, you will be a key member of our UCR service, providing in-depth assessments of nursing care and treatment for patients in their own homes. Your expertise will help prevent unnecessary admissions and facilitate timely discharge from hospital.

You will work closely with a team of experienced clinicians, utilizing your advanced clinical skills, non-medical prescribing, and case management approach to deliver high-quality patient care. Your ability to work effectively in a community setting, with a focus on patient-centered care, will be essential to the success of this role.

Requirements
  • Independent non-medical prescriber with demonstrable experience
  • Advanced physical assessment and disease management skills (level 7) or willingness to work towards this qualification within the first 2 years of appointment
  • Teaching and mentoring qualification
  • Diploma or degree awarded by a professional body
  • Registered as fit to practice with the Nursing and Midwifery Council (NMC) as a registered adult nurse
  • Professional knowledge acquired through degree, supplemented by postgraduate diploma specialist training, experience, and further specialist training
