Job Title: Discharge Coordinator

Job Summary:

Livewell Southwest is seeking a skilled Discharge Coordinator to join our team. As a Discharge Coordinator, you will play a vital role in supporting individuals with complex health and social needs to achieve a smooth transition from hospital to community care.

Key Responsibilities:

  • Develop and implement discharge plans in collaboration with multidisciplinary teams, patients, and their families.
  • Identify and address social, housing, and financial barriers to discharge.
  • Collaborate with local authorities, community services, and other stakeholders to ensure seamless transitions.
  • Monitor patient progress, bed availability, and delayed discharges to prevent unnecessary delays.
  • Provide support and guidance to patients, families, and carers to empower them to make informed decisions.

Requirements:

  • NVQ Level 5 or equivalent qualification in a relevant field.
  • Experience working with individuals with mental health diagnoses and complex needs.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize tasks.
  • Knowledge of local processes for bed management and community resources.
