What We Seek
Challenges related to housing and living circumstances can significantly affect mental health and overall wellness. We are currently searching for Community Support Facilitators to partner with District and Borough housing teams. Our Support Facilitators are well-versed in their communities, aware of the array of services available, and committed to ensuring individuals receive the necessary assistance right in their neighborhoods.
By collaborating closely with clients, local councils, housing associations, healthcare professionals, and service providers, Support Facilitators aim to enhance the journey towards improved living conditions and overall well-being. They connect individuals with existing resources, contribute to the development of new local services, work alongside healthcare professionals, track progress, and ensure that the positive outcomes of our services are clearly evidenced - all of which genuinely transforms lives.
This position is well-suited for individuals with a background in community mental health and wellness, employing a case management approach. The ideal candidate will be solution-focused, organized, capable of meeting service demands, and ready to engage with local authorities.
Key Qualifications:
Applicants should possess:
- A minimum SVQ/NVQ level 3 qualification
- Experience in implementing mental health interventions with an emphasis on case management
- Experience in assisting clients facing diverse challenges in health and social care environments
