Community Outreach Specialist

About the Role
We are seeking a skilled and compassionate Community Link Worker to join our team at Shaw Trust. As a Community Link Worker, you will play a vital role in supporting individuals with complex needs, connecting them with local services, and promoting their overall wellbeing.
Key Responsibilities
- Deliver mental health and wellbeing interventions in a case management approach, working closely with clients, district and borough councils, housing associations, and health professionals.
- Facilitate and support clients with complex and/or multiple barriers to success in health and social care settings.
- Broker existing services and help develop new local provision to ensure a person's journey to better living circumstances and wellbeing is as easy as possible.
Requirements
- Minimum of SVQ/NVQ level 3
- Experience of delivering mental health and wellbeing interventions in a case management approach
- Experience of facilitating and supporting clients with complex and/or multiple barriers to success in health and social care settings
