About the Role

Shaw Trust is seeking a skilled Community Link Worker to join our team. As a Community Link Worker, you will work closely with district and borough housing teams to address issues related to an individual's housing or living circumstances that can impact their mental health and wellbeing.

Key Responsibilities

  • Work with clients, district and borough councils, housing associations, health professionals, volunteers, and service providers to ensure a person's journey to better living circumstances and wellbeing is as easy as possible.
  • Broker existing services and help develop new local provision to support individuals with complex and/or multiple barriers to success in health and social care settings.
  • Liaise with primary and secondary health care professionals to ensure effective support and check in on progress to make sure we can evidence the outcomes of the service.

Requirements

  • Minimum of SVQ/NVQ level 3
  • Experience of delivering mental health and wellbeing interventions in a case management approach
  • Experience of facilitating and supporting clients with complex and/or multiple barriers to success in health and social care settings
