About the Role

The Community Response Team provides short-term/interim enablement support to individuals who have been discharged from acute hospitals or experience an acute change in their function. As a Community Response Support Worker, you will work closely with clinicians and other healthcare professionals to deliver agreed programmes of treatment and care.

Key Responsibilities
  • Provide high-quality care and support to patients in their own homes
  • Work closely with clinicians and other healthcare professionals to deliver agreed programmes of treatment and care
  • Assist with the development of individual care plans and ensure their implementation
  • Document patient care and progress accurately and in a timely manner
  • Travel to patients' homes across the county, using your own vehicle if necessary
Requirements

To be successful in this role, you will need:

  • NVQ Level III in care or equivalent
  • Experience of working in a healthcare setting
  • Good verbal communication skills and the ability to work effectively with patients, families, and healthcare professionals
  • Able to travel across the county and work flexibly, including evenings and weekends
