Clinical Team Lead Community Nursing Specialist

We are seeking a highly skilled and experienced District Nurse Team Manager to lead our team of community nurses and healthcare assistants in providing high-quality care to patients in their homes.
The successful candidate will have a strong background in community nursing, with experience in managing a team and leading clinical practice. They will be responsible for developing and implementing care plans, managing caseloads, and ensuring that patients receive the best possible care.
The role will involve working closely with other healthcare professionals, including GPs, physiotherapists, occupational therapists, and social workers, to ensure that patients receive comprehensive care. The successful candidate will also be responsible for monitoring and supporting staff wellbeing, ensuring that staff maintain competencies and receive ongoing education and training.

About the Role- Lead a team of community nurses and healthcare assistants in providing high-quality care to patients in their homes.
- Develop and implement care plans, manage caseloads, and ensure that patients receive the best possible care.
- Work closely with other healthcare professionals to ensure that patients receive comprehensive care.
- Monitor and support staff wellbeing, ensuring that staff maintain competencies and receive ongoing education and training.
- Registered level 1 Nurse.
- Specialist qualification in District Nursing or postgraduate diploma/degree level study related to health.
- Recognised Management qualification or courses e.g., ILM course, or leadership, management modules, core management study days, in-house courses (HR, budgets, appraisals, staff performance, sickness).
- Ability to travel across the city.
