Key Responsibilities:
- Operate within the defined clinical practice framework to serve as the initial point of contact for patients.
- Conduct in-person, telephonic, and virtual consultations, including the management of patients with chronic conditions.
- Gather medical histories, perform physical assessments, update patient records, establish diagnoses, and provide treatment plans.
