We are seeking a highly skilled and compassionate Mental Health Practitioner to join our Community Mental Health Team for Older People in Weymouth. As a key member of our team, you will work closely with patients, carers, and other professionals to provide person-centered care and support.
Key Responsibilities- Assess, plan, implement, and evaluate programs of care for patients with complex mental health needs.
- Develop and maintain effective relationships with patients, carers, and other healthcare professionals.
- Contribute to the development and implementation of care plans that promote patient independence and well-being.
- Participate in multidisciplinary team meetings to discuss patient care and share best practices.
- Collaborate with other healthcare services to ensure seamless care transitions and optimal patient outcomes.
- Registered Mental Nurse with valid registration with the NMC.
- Experience working with older adults with complex mental health needs.
- Strong assessment and planning skills, with the ability to prioritize and manage complex cases.
- Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
- Competent computer skills, with experience using electronic patient records.
