Older Adult Community Mental Health Nurse/Practitioner

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our team in Weymouth, working with older adults who require specialist mental health care. As a key member of our Community Mental Health Team, you will provide person-centered, holistic care to support the wellbeing and recovery of our patients.
Key Responsibilities- Act as a dedicated Care Coordinator for a caseload of patients, providing comprehensive assessments, treatment plans, and ongoing support.
- Work collaboratively with carers, other professionals, and community services to ensure seamless care coordination.
- Develop and implement effective risk assessment and management strategies to ensure patient safety.
- Participate in duty rota, working with a dynamic team to provide 24/7 coverage and support.
- Contribute to the development of the team's skills and knowledge through ongoing learning and professional development opportunities.

Requirements- Registered Mental Nurse with valid NMC registration.
- Experience working with older adults with mental health needs, preferably in a community setting.
- Strong assessment, planning, and implementation skills, with a focus on person-centered care.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with diverse stakeholders.
- Competent computer skills, with experience using electronic patient records.
