Older Adult Community Mental Health Nurse/Practitioner Position

We are seeking a skilled Community Mental Health Practitioner to join our team in Weymouth, working with older adults and providing person-centered care.
The ideal candidate will have experience working with individuals suffering from severe mental illness and be able to assess, plan, implement, and evaluate programs of care.
The role involves working as part of an integrated multi-disciplinary team, providing holistic care to patients and assisting in their recovery.
The successful candidate will have excellent communication skills, be able to work collaboratively with carers and other professionals, and have a positive approach to delivering high-quality care.

Key Responsibilities- Assess, plan, implement, and evaluate programs of care for older adults
- Work collaboratively with carers and other professionals to provide holistic care
- Develop and maintain effective relationships with patients, carers, and other stakeholders
- Contribute to the development of the team and the service
- Registered Mental Nurse with valid registration with NMC
- Evidence of continuous professional development
- Valid RMN registration
- Competent computer skills, experience inputting information on RIO
