Leaders in Care is seeking a dedicated and compassionate Social Worker to join our team in providing specialist advice and assessment to elderly service users with existing or developing mental health issues.

  • Assess the needs of older adults and develop individualized care plans
  • Provide support to older adults and their families

Requirements
