About the Role

We are seeking a skilled and dedicated Adult Nurse to join our community team at Sussex Community NHS Foundation Trust. As a Community Nurse, you will be responsible for providing high-quality, patient-centered care to individuals in their own homes.

Key Responsibilities
  • Conduct holistic assessments and develop care plans for patients in their own homes.
  • Provide individualized treatment and promote self-care.
  • Develop and maintain a wide range of clinical skills, including wound care, palliative care, and medicines management.
  • Work collaboratively with other healthcare professionals to ensure seamless care delivery.
  • Participate in clinical governance, clinical audit, and clinical supervision to maintain high standards of care.
Requirements
  • Registered Adult Nurse with NMC registration.
  • Experience of community nursing, with a commitment to working in this setting.
  • Ability to assess, develop, implement, and evaluate programs of care.
  • Knowledge of current local policies informing community health and social care.
  • Car driver and access to a car for work purposes.
