We are seeking an experienced Mental Health Practitioner to join our Assertive Transition Service (ATS) team. As a key member of our multidisciplinary team, you will play a vital role in supporting the safe and smooth discharge of forensic mental health patients from secure hospital settings into the community.
Key Responsibilities- Assess patient needs and develop treatment plans within a multi-disciplinary framework.
- Work collaboratively with colleagues from multiple agencies to ensure a robust package of care is available.
- Develop and maintain partnerships with Community Forensic Services, Adult Mental Health Services, Private Sector, and Local Authorities.
- Deliver person-specific interventions, including training packages.
- Have a sound knowledge of Mental Health legislation, including the Mental Health Act, Mental Capacity Act, and Deprivation of Liberty Safeguards.
- Experience of working in or closely with a multi-disciplinary team in Community or Forensic Mental Health.
- Previous experience working with adults who have complex needs.
- Demonstrate an understanding of the Care Programme Approach for clients in the community.
- Able to demonstrate successful multi-disciplinary working.
- Experience of leading clinical assessments and providing high-quality feedback.
- Demonstrate working knowledge of Mental Health Act, MCA, and DOLS Forensic Sections.
- Awareness of different assessment tools available, such as the HCR-20v3.
- Experience of working in Forensic Services.
- Excellent report writing skills.
- Positive and effective communication skills.
- Ability to be flexible and handle conflict.
- Ability to promote a clinical perspective within a multi-agency forum.
- Assessment skills.
