Job Summary

University Hospital Southampton NHS Foundation Trust is seeking a highly motivated and experienced individual to join our Diabetes and Endocrinology team as a Clinical Support Practitioner. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality patient care and contributing to the development of our service.

Main Duties

The successful candidate will work as part of the Diabetes and Endocrinology multidisciplinary team, providing specialist nursing care and education to patients with diabetes and potential endocrine conditions. Key responsibilities will include:

  • Providing phlebotomy services and dynamic function testing in accordance with trust protocols and guidelines.
  • Undertaking observations, recognizing changes, and escalating to the Endocrine Specialist Nurses as required.
  • Ensuring accurate and timely documentation in patient medical records.
  • Administering prescribed medication in a safe and timely manner, in accordance with trust policies and training.
  • Delivering direct patient education, including steroid education, blood glucose monitoring, and insulin administration.
  • Promoting effective communication with patients, relatives, and the multidisciplinary team.
  • Liaising with Primary and Secondary care colleagues to facilitate safe handover of patient care plans.
  • Contributing to service development, delivering high standards of care, and adhering to National and Local targets and guidelines.

Person Specification

The ideal candidate will possess:

  • A Foundation Degree in Health and Social Care or equivalent.
  • Basic Maths, English, and IT skills.
  • Experience of working in a healthcare setting in a clinical supporting role.
  • Competence in the use of manual blood pressure, pulse, oxygen saturation, and weight, with the ability to record and interpret measurements.
  • Excellent interpersonal, verbal, and written communication skills.
  • General IT skills, including the use of the Microsoft Office suite.
  • Experience in medicines administration and venepuncture/cannulation.
  • Excellent time management and organizational skills.
