We are seeking a highly motivated and enthusiastic individual to join our team as an Associate Practitioner in Diabetes and Endocrinology at University Hospital Southampton NHS Trust.
About the RoleThe successful candidate will work as part of the diabetes and endocrine multidisciplinary team, providing specialist nursing care and education to patients with diabetes and potential endocrine conditions. The role will involve working alongside the Diabetes and Endocrine Multidisciplinary team in the provision of patient care, undertaking phlebotomy and dynamic function testing, and ensuring accurate and timely documentation in patients' medical records.
Key Responsibilities- Work alongside the Diabetes and Endocrine Multidisciplinary team in the provision of patient care.
- Undertake phlebotomy and dynamic function testing in accordance with trust protocols and guidelines under the supervision of the Endocrine Specialist Nurses.
- Ensure accurate and timely documentation in patients' medical records.
- Administer prescribed medication in a safe and timely manner in accordance with trust policies and training.
- Deliver direct patient education, including steroid education, blood glucose monitoring, and insulin administration.
- Promote effective communication with patients, relatives, the MDT, and deal with inquiries in a professional manner.
- Liaise with Primary and Secondary care colleagues to facilitate the safe handover of patient care plans.
- Actively contribute to service development, delivering high standards of care in accordance with National and Local targets and guidelines.
- Work independently under the guidance of a registered nurse, acknowledging own skills and boundaries, and taking opportunities to develop self-knowledge.
- Deliver health promotion advice to patients under the care of the UHS diabetes and endocrinology service.
- Work with clinical leads to collect evidence of benefit in the role to secure continuation of funding for the post/role.
- Check stock levels of equipment required by the diabetes and endocrinology specialist nurses and order as appropriate.
This is a part-time role, working 7.5 hours a day, Tuesday to Thursday, between 8am and 5pm.
Person Specification- Nursing Associate on the NMC Register.
- Assistant Practitioner with a health-related Foundation Degree, e.g. Foundation Degree in Health and Social Care.
- Basic Maths, English, and IT skills.
- Experience of working in a healthcare setting in a clinical supporting role.
- Competent in the use of manual blood pressure, pulse, oxygen saturation, and weight, able to record and interpret measurements.
- Excellent interpersonal, verbal, and written communication skills.
- General IT Skills, including the use of the Microsoft Office suite.
- Experience in medicines administration.
- Venepuncture/cannulation.
- Excellent time management and organisational skills.
- Experience of working in an MDT across organisational and professional boundaries.
- Experience in providing patient education or training.

Salary: £26,530 to £29,114 a year (Pro rata)
