Positive Intervention Practitioner/Social Care Specialist

As a Positive Intervention Practitioner/Social Care Worker with Choice Care Group, you will play a vital role in providing high-quality care and support to individuals with learning disabilities, autism, and mental health conditions.
Key Responsibilities- Deliver person-centred care, including bathing, toileting, dressing, and feeding, in accordance with individual care plans and company policies.
- Follow care plan guidance and company procedures to ensure the highest standards of care and support.
- Attend and complete training as required to enhance your skills and knowledge.
- Participate in the development and implementation of Positive Behaviour Support plans to reduce challenging behaviours.
- Contribute to the development of interventions aimed at reducing the intensity and frequency of challenging behaviours.
- Offer individual and group work to individuals receiving Choice Care services, under the supervision of the Clinical Lead and Senior Practitioner.
- Facilitate the smooth implementation of care plan systems within the homes.
- Be patient, reliable, flexible, a team player, and a good communicator.
- Hold the Learning Academy - Adult Care Worker Level 2 and Lead Adult Care Worker Level 3 qualifications.
