Positive Intervention Practitioner/Social Care Specialist

About the Role:

Choice Care is seeking a dedicated and compassionate Positive Intervention Practitioner/Social Care Specialist to join our team. As a key member of our care team, you will provide high-quality care and support to individuals with learning disabilities, autism, and mental health conditions.

Key Responsibilities:

  • Provide personal care, including bathing, toileting, dressing, and assistance with feeding.
  • Follow care plan guidance and adhere to company policies and procedures.
  • Attend and complete training as required.
  • Assist in the development of interventions aimed at reducing challenging behaviors.
  • Offer individual and group work to individuals receiving Choice Care services.
  • Facilitate the smooth implementation of care plan systems within the homes.

Requirements:

  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
  • Flexibility and willingness to work varied shifts, including weekends.
  • Basic knowledge of care planning and behavior support.
  • Ability to maintain confidentiality and adhere to data protection policies.
