About the Role

We are seeking a dedicated and compassionate Care Practitioner to join our team at Choice Care. As a Care Practitioner, you will play a vital role in providing high-quality care and support to individuals with learning disabilities, autism, and mental health conditions.

Key Responsibilities
  • Provide personal care, including bathing, toileting, dressing, and assistance with feeding
  • Follow care plan guidance and adhere to Company policies and procedures
  • Attend and complete training as required
  • Assist in the development of interventions to reduce challenging behaviors
  • Offer individual and group work to people receiving Choice Care services
  • Facilitate the implementation of Care Plan systems within the homes
