Job Summary:

We are seeking a highly motivated and enthusiastic Registered Nurse to join our dynamic specialist team at St Luke's Hospice Plymouth as a Clinical Nurse Specialist in Palliative Care.

Key Responsibilities:

  • Manage patients with complex holistic needs and provide specialist palliative care to meet their practical, psychological, spiritual, and social needs.
  • Work closely with primary and secondary care professionals and Hospice colleagues to ensure seamless care delivery.
  • Develop and implement individualized care plans to address the unique needs of patients and their families.
  • Provide education and support to patients, families, and healthcare professionals to promote palliative care best practices.
  • Contribute to the development and implementation of palliative care policies and procedures.

Requirements:

  • Registered Nurse with a strong background in palliative care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong problem-solving and critical thinking skills.
