We are seeking a highly organized, proactive, and motivated individual to take on the role of Lead Support Practitioner within our Supported Living Service. This service comprises multiple residences and provides assistance to individuals with learning disabilities and/or autism. The individuals we support may also have additional requirements such as challenging behaviors, mental health issues, and complex needs. Our approach focuses on empowering individuals to lead fulfilling lives, maintain their independence, engage with their community, develop essential life skills, and reach their personal aspirations.

As a Lead Support Practitioner, you will engage with individuals in a variety of enjoyable activities, such as attending social events, swimming, and visiting cinemas. In addition to delivering direct care and support, you will have the chance to serve as a Key Worker, guiding individuals in achieving their goals and dreams.

Key Responsibilities:

Ensure that all service users receive high-quality, personalized, and person-centered care Provide mentorship, support, and guidance to the staff team within the service Foster warm and trusting relationships with clients, encouraging them to communicate their needs, opinions, and concerns Assist in managing service delivery to effectively meet client needs Flexibility in working hours is required, including evenings, weekends, waking nights, and sleep-ins

A minimum of two years' experience in providing care and support to individuals with learning disabilities and other needs is essential for this position, along with a strong commitment to delivering exceptional managerial support to a dedicated staff team.
