Key Responsibilities:

  • Develop and maintain empathetic, professional relationships with individuals, their caregivers, and families.
  • Collaborate with fellow practitioners, partner organizations, and community resources to enhance service delivery.
  • Utilize timely and suitable social work interventions tailored to the needs of individuals and their support networks.
  • Conduct thorough, person-centered assessments to understand the unique circumstances of each individual.
  • Co-create comprehensive support plans that focus on achieving positive outcomes.
  • Manage a diverse caseload of complex situations, prioritizing tasks effectively.
  • Address crisis situations promptly and efficiently, participating in the service Duty system as necessary.
  • Deliver high-quality, responsive services by applying a broad range of social work theories and methodologies.
  • Make informed decisions regarding safeguarding issues and lead inquiries in accordance with established policies.
  • Engage with individuals using various communication methods to understand their needs and preferences.
  • Ensure adherence to anti-discriminatory practices in line with relevant legislation.
