Job Summary

This is an exciting opportunity for a highly motivated and experienced nurse to join our community-based team of Heart Failure Specialist Nurses delivering advanced practice care to patients with chronic heart failure in a rural district area.

Main Responsibilities

The post holder will manage an identified group of patients with complex health and social care needs, using a case management model. Combining highly specialist nursing knowledge and advanced skills to proactively manage patients with Heart Failure, to improve the quality, experience, and choice about their care, enabling them to remain at home longer.

Please see the attached Job Description and Person Specification. The post holder must have at least 18 months of experience managing chronic heart failure patients and have completed a Heart Failure module. They will also be an Independent Prescriber.
