Your responsibilities will include genuine multidisciplinary teamwork, fostering connections with general practice, care facilities, community rapid response teams, and fall prevention services to ensure effective daily service delivery.

North Eastern Derbyshire Healthcare Limited is in search of Senior Clinical Practitioners and Trainee Clinical Practitioners to collaborate with Community General Practitioners and other multidisciplinary team members to provide healthcare to patients who are housebound or residing in care facilities. This service is continuously evolving, offering you the chance to influence the development of the Collaborative Care initiative and the Home Visiting Service. Trainee Clinical Practitioners will receive guidance from the team’s Community General Practitioners, Clinical Lead Practitioners, and experienced practitioners, along with support throughout their educational journey.

Key Responsibilities

North Eastern Derbyshire Healthcare Limited is looking to recruit Senior Clinical Practitioners and Trainee Clinical Practitioners to work within a multidisciplinary team, delivering care to housebound individuals and those in care or residential settings.

The primary objective of this service is to provide a holistic approach to acute and rapid response services, enhanced health management in care homes, and proactive care for older adults experiencing frailty and complex health issues.

Job Duties

Trainee Clinical Practitioners will either be working towards meeting the Essential Criteria outlined in the person specification or will be committed to achieving these standards.

Qualifications

Essential

  • Registered Nurse - with RCN/NMC registration
  • Paramedic - Registered with HCPC, with an HCPC recognized qualification or on the Paramedic Practitioner Pathway
  • MSc in Advanced Clinical Practice (or equivalent)
  • Evidence of post-registration education at undergraduate level (Level 6)
  • Independent Prescribing or completion of Pharmacology and Therapeutics at postgraduate level (Level 7)
  • Experience in mentoring and coaching

  • Teaching qualification

Experience

  • Autonomous practitioner
  • Experience managing long-term conditions, older adults with complex needs, and end-of-life care
