Main Responsibilities:- Clinical Leadership:
- Provide clinical leadership to the PCN's Pharmacy team, identifying areas of service improvement and best practice.
- Develop a Pharmacy clinical strategy in conjunction with the PCN General Manager and PCN Clinical Director.
- Work collaboratively with our multi-disciplinary team to improve patient outcomes and reduce health inequalities.
- Team Management:
- Manage and mentor a team of Pharmacists and Pharmacy Technicians.
- Service Development:
- Develop, implement, and evaluate clinical protocols and integrated care pathways within the area of expertise or specialty.
- Collaborate with other relevant professionals to develop clinical protocols and integrated care pathways.
- Support and Advice:
- Provide high-level pharmaceutical support, advice, and information to medical, nursing, and other healthcare staff providing clinical care in General Practice.
- Clinical:
- Support PCN Practices in the delivery of chronic disease management, structured medication reviews, and proactive management of patients with complex polypharmacy.
- Be actively involved in the management of patients with long-term conditions.
- Utilize independent prescribing qualification in accordance with personal competencies.
- Manage medication reviews and safe and effective prescribing in care homes.
- Provide patient-facing and telephone support to patients with queries and concerns regarding their medication.
- Implement changes required regarding MHRA Alerts and local and national guidance.
- Provide advice and recommendations on shared care, NICE guidance, and RAG lists.
- Manage discharge summaries from hospitals and community services.
- Provide high-level pharmaceutical support, advice, and information to medical, nursing, and other healthcare staff providing clinical care in General Practice.
- Leadership:
- Demonstrate an expert understanding of Pharmacy in General Practice and PCNs.
- Provide clinical line management to the PCN Pharmacy team, empowering, developing, and valuing all colleagues.
- Effectively and professionally represent the PCN Pharmacy team by providing necessary service updates at both internal and external meetings.
- Collaborate with other clinical leads to facilitate the delivery of training for the multi-disciplinary team and PCN member practices.
- Support colleagues to advance clinical skills or develop new roles.
- Assume responsibility for own continuing professional development in relation to role and contribute to the continuing professional development of other healthcare professionals.
- Undertake annual mandatory training updates and other relevant courses in line with organisational and local policies.
- Facilitate and promote a learning culture that encourages others to develop their full potential.
- Communicate, motivate, and inspire others to deliver excellent standards of care.
- Engage in the appraisal process and act as appraiser of others as appropriate.
- Provide mentorship and/or clinical supervision to staff and learners to aid the application of theory to practice.

- PCN and Pharmacy Service Development:
- Utilize the expertise of the entire PCN Pharmacy team to develop and implement a Pharmacy Clinical Strategy which improves outcomes for patients and reduces health inequalities.
- Identify and proactively capitalize on service improvement opportunities which will be of benefit to the PCN, its member Practices, and their patients.
- Understand and contribute to the delivery of the PCN DES contractual requirements and targets.
- Foster and maintain strong links with all relevant services across our locality, looking for opportunities to work collaboratively with system partners, PharmacyFirst.
- Manage the quality-of-service delivery through clinical audit, incident reporting, Significant Events Analysis, and standard setting.
- Implement recommendations from local clinical audits with support from the MDT Clinical Leads.
- Work with member Practices to develop new and innovative ways of working which enable the PCN to thrive through the sharing of best practice and working collaboratively.
- Play an integral role in delivering the relevant CQC and Quality Schedule requirements.
- Information Resources / Information Systems:
- Analyse and interpret highly complex information through use of the Clinical System.
- Evaluate the quality of own work and current practices using evidence-based practice projects, audit, and outcome measures and support others in doing so.
- Develop benchmarks for best practice, identify quality outcome measures, and contribute to audit programmes.
