Job Summary

We are seeking a highly skilled and motivated Pharmacy Technician to join our established pharmacy team at North Eastern Derbyshire Healthcare Limited. As a Pharmacy Technician, you will play a vital role in supporting our Clinical Pharmacists and Practice Nurses in delivering high-quality patient care and improving health outcomes.

Main Responsibilities

The successful candidate will be responsible for:

  • Undertaking patient-facing and patient-supporting roles to ensure effective medicines use through shared decision-making conversations with patients.
  • Carrying out medicines optimisation tasks, including effective medicine administration, supporting medication reviews, and medicines reconciliation.
  • Utilising consultation skills to work in partnership with patients to ensure they use their medicines effectively.
  • Supporting medication reviews and medicines reconciliation for new patients.
  • Synchronising medicines for patient transfers between care settings, linking with local community pharmacies.
  • Supporting the Clinical Pharmacist in Structured Medication Reviews (SMR) and organising necessary monitoring tests prior to SMR.
  • Providing expertise to address both public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.
  • Managing shared care protocols and liaising with Clinical Pharmacists for more complex patients.
Technical and Administrative

The successful candidate will also be responsible for:

  • Leading on clinical searches and monitoring of data.
  • Participating in developing the PCN Pharmacy team in respect to developing policies and procedures.
  • Contributing to the design and delivery of patient questionnaires or pre-review resources to refine the system for medication reviews.
  • Collating and filtering information from the patient to be documented on the clinical system or escalated to a Clinical pharmacist or other relevant clinician in the practice, if appropriate.
  • Supporting the practice Prescription's teams in streamlining general prescription requests, to allow GPs and clinical pharmacists to review the more clinically complex requests.
  • Supporting the implementation of local and national prescribing policies and guidance within GP practices, care homes, and other primary care settings.
  • Supporting the Primary Care Network to deliver on contractual agenda, support Practices with QOF and locally commissioned enhanced services.
