About the Role

We are seeking a dedicated and compassionate Peripatetic Support Assistant to join our team at Wakefield Metropolitan District Council. As a Peripatetic Support Assistant, you will play a vital role in providing high-quality care and support to individuals in their own homes, promoting independence and delivering a person-centred service.

Key Responsibilities
  • Deliver person-centred care and support to individuals in their own homes, promoting independence and dignity.
  • Work collaboratively with other professionals and agencies to deliver effective services, adhering to national guidelines and standards.
  • Contribute to the development and implementation of individual care plans, ensuring that service users receive the best possible care.
  • Participate in regular team meetings and contribute to the development of the Peripatetic Service.
  • Attend training and development opportunities to enhance skills and knowledge.

Requirements
  • The right to work in the UK.
