About the Role

Key Responsibilities
  • Deliver person-centred care and support to service users, promoting independence and dignity.
  • Work collaboratively with other professionals and agencies to ensure effective service delivery.
  • Contribute to the development and implementation of individual care plans.

Requirements
  • Ability to work a 4-week rota, including evening shifts and weekends.
  • Willingness to undergo comprehensive training and induction programme.
