Position Title: Social Worker

Department: Neighbourhood Team

Working Hours: 37 hours per week

Salary Grade: H (£35,745 - £39,186 per annum, equivalent to £18.52 to £20.31 per hour)

Employment Type: Fixed Term Contract for 12 months

Role Requirements: A full, clean driving licence is essential. Candidates must possess a Social Work degree and be registered with the Social Work England.

Job Overview

An exciting opportunity has emerged for a Social Worker to join the Bolton neighbourhood team, focusing on adults and older adults in the west/central north region.

As a Social Worker in this team, you will collaborate with multidisciplinary colleagues to deliver a cohesive approach to fulfilling the needs of individuals within our communities.

Key responsibilities include:

  • Conducting assessments of need in accordance with the Care Act 2014.
  • Developing and implementing strengths-based support plans.
  • Assisting service users in accessing universal and voluntary services.
  • Performing assessments and carers assessments, as well as formulating support plans under the Care Act 2014.

Knowledge and experience in adhering to the principles of the Mental Capacity Act 2005 and other relevant Social Work legislation and guidance are crucial.

The role also involves participating in a busy duty system on a rota basis, managing referrals, and providing information and advice at the point of contact.

Preferred Experience:

  • Managing an active caseload.
  • Conducting safeguarding investigations.
  • Participating in continuing health care assessments and multi-agency meetings.
  • Engaging in multidisciplinary teamwork.

Candidate Profile

We seek an enthusiastic and motivated individual with a genuine passion for Social Work practice and experience working with adults in community settings. The ideal candidate will possess the ability to foster, maintain, and enhance relationships with service users and carers, as well as the skills necessary for effective interdisciplinary collaboration.

These positions require enhanced clearance from the Disclosure and Barring Service and registration with Social Work England.
