As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and supporting the development of our healthcare services.
Key Responsibilities- Provide expert clinical care to patients with a range of acute and chronic medical conditions, utilizing your advanced practice skills and knowledge to optimize patient outcomes.
- Work collaboratively with our multidisciplinary team to identify and manage patients at risk of developing long-term conditions, implementing evidence-based interventions to prevent adverse effects on patient health.
- Develop and maintain accurate clinical records, ensuring compliance with relevant legislation and guidelines.
- Prioritize health issues and intervene appropriately, recognizing and assessing patients presenting with mental health needs and referring them to specialist services as required.
- Support patients in the use of their prescribed medications, reviewing their medication regimens annually to ensure optimal treatment plans.
- Contribute to the development of our primary care team, providing guidance and support to colleagues to enhance their skills and knowledge.
- Registered Healthcare Professional with a postgraduate diploma or degree in Advanced Practice or a related field.
- Experience of working in a primary care environment, with a strong understanding of the challenges and opportunities presented by this setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, colleagues, and other stakeholders.
